Respecting Choices“ materials and handouts
include the following:
1. Registration for 30 people for the Respecting Choices
On-line Facilitator Course
2. Respecting Choices“ Advance Care Planning Facilitator's
Manuals: number to be determined by need and request
3. Respecting Choices“ Organization & Community
Advance Care Planning Course Manual (for those attending
Day 3)
4. A complimentary sample of Gundersen Lutheran's patient
educational materials
5. Extensive bibliographies and references
6. Those individuals who participate in the Instructor
Certification Component (Day 3) will receive:
a. Respecting Choices“ Instructor's Guide with
a complete set of materials to teach the Respecting
Choices“ Advance Care Planning Course for Facilitators,
to include instructor's notes for each training module;
role-play and problem solving scenarios, three videos
(Making Choices“ and Advance Care Planning with
a Healthy Adult; Advance Care Planning with Chronic
Progressive Illness); slides and program brochure
template.
b. Agreement for permission to replicate the Respecting
Choices“ Advance Care Planning Course for Facilitators
in their community.
The sponsoring organization will be responsible for
making all arrangements to include: facilities, AV
equipment, marketing, communication and registration
of participants, submitting a registration database
to Respecting Choices“, beverages and food,
continuing education credits as needed
